If you have received notice of a continuing education audit please refer to the following guidelines when submitting your verification:

• Each verification document must include the following information: your name, the name of the course, the date of the course, the sponsor of the program and the number of hours awarded.

• Please submit copies of your verification documents. Do not submit original documents, as these will not be returned.

• Documents must be submitted in date order, as shown on the CE Report. If they are not submitted in that order they will be returned to you and you will be required to resubmit in the correct order.

• For CPR/ACLS courses only: a copy of your card will be accepted as veriifcation. Expired CPR/ACLS cards do not need to be submitted as we have a copy on file.

• If you attended the SD Dental Association Annual Meeting please provide a copy of your name badge AND the code sheet. You do not need to verify the exhibits.

• If you attended a course that provides codes at the end of each speaker, the code sheet will be accepted as proof of attendence only if it contains the information noted above in the first bullet point.

• If you are missing an attendance document for a course, contact the sponsor of that course to obtain a copy. If satisfactory verification of attendance cannot be produced, continuing education hours may be deducted from the total hours reported.

• Receipt of payment is not verification of attendance for any course.

All documentation must be mailed to the Board office by the deadline noted in the letter. Do not fax or email this documentation. If you are unable to meet the deadline, please contact our office to discuss an extension. Please allow 4-6 weeks after the audit deadline for our office to process the audit paperwork. Upon completion of the audit you will be notified in writing.
